How Much Does Post-SAP Follow-Up Testing Cost? (The Fee Nobody Advertises)

Post-SAP follow-up testing has two costs: a program fee for someone to manage your SAP’s plan, and a per-test price for each directly observed test. At Vertical Identity the program fee is $240 per year and each observed return-to-duty or follow-up test is $119, which is the $69 base DOT drug test plus a $50 observation surcharge. Your SAP sets how many tests you take, with a federal minimum of six in the first 12 months, so the per-test price matters more than any headline number.

Last updated October 12, 2026. Vertical Identity prices are current as of this date; competitor prices are per their published pages as of July 2026.

Key takeaways

  • Every return-to-duty and follow-up urine collection must be directly observed under 49 CFR §40.67, so an “observed collection” add-on applies to 100% of your tests.
  • Six observed follow-up tests at Vertical Identity cost $714. Add the $240 program fee and the $119 return-to-duty test and a typical first year comes to $1,073.
  • The SAP decides how many tests you take. The minimum is six in the first 12 months, and the plan can run up to 60 months in total.
  • The SAP evaluation is a separate cost paid to the Substance Abuse Professional. It is not part of testing or program fees.
  • When you compare providers, compare the observed per-test price and the annual program fee, not the advertised test price.

What does post-SAP follow-up testing cost in total?

Nobody can quote a fixed total in advance, because the SAP (Substance Abuse Professional) prescribes the number of tests, not the testing provider. What you can add up is the pieces:

  1. The SAP evaluation and any treatment or education the SAP assigns. These are paid to the SAP and treatment providers. They are separate from testing.
  2. Program management. Someone has to hold the SAP’s follow-up plan, schedule unannounced tests, route you to a collection site, and keep the records. Providers charge this as an annual fee, a one-time setup fee, or not at all in a published price.
  3. The return-to-duty test. One directly observed test, passed with a verified negative result, before you can go back to safety-sensitive work.
  4. The follow-up tests. A minimum of six unannounced, directly observed tests in your first 12 months back on duty, and possibly more for up to 60 months total, as your SAP directs.

Items 3 and 4 are where the spread between providers shows up, because each test carries the observation question: is observation in the price, or added on top?

Why are follow-up tests more expensive than the advertised price?

Under 49 CFR §40.67, return-to-duty and follow-up urine collections must be directly observed. An unobserved follow-up test does not exist as an option, and a driver cannot decline observation.

Because observation is mandatory, any “observed collection” add-on listed separately on a rate card applies to every test you take. The headline price is not the price you pay.

Here is a competitor example, per goMDnow’s published pricing as of July 2026 (verify when you buy):

Listed itemPrice
Follow-up DOT urine drug test, non-member$89.95
Follow-up DOT urine drug test, consortium member$79.95
Observed collection add-on (“Required for RTD and certain directly observed DOT collections”)+$45.00
Real cost per follow-up test$134.95 non-member / $124.95 member

The member rate also requires a separate $99.00 per year consortium membership. A $134.95 follow-up enrollment fee (plan setup $99.95 plus Clearinghouse reporting $35.00) is listed on top of that.

One coincidence confuses people: the enrollment fee and the non-member per-test cost are both $134.95, but the first is charged once and the second is charged every time you test.

At Vertical Identity the observed test is $119, made up of the $69 base DOT drug test and a $50 observation surcharge. It is one number on the page and one number on the invoice.

What are you buying: a managed program or à la carte tests?

Two structures sit behind the prices, and they are different products:

  • À la carte. You pay a setup fee, then order each test as your SAP’s schedule requires. You or your employer still track the calendar and make sure nothing is missed.
  • Managed program. A C/TPA (consortium/third-party administrator) holds the SAP’s plan, builds the unannounced schedule, tells you when and where to test, handles medical review officer (MRO) review, files Clearinghouse reports, and keeps audit-ready records.

Comparing a setup fee to an annual management fee compares two different things. If you buy an enrollment product and are still the one watching the calendar, you bought paperwork, not management. A missed test is treated as a refusal under 49 CFR §40.191, which has the same consequences as a positive result, so who tracks the schedule is a real cost question.

Ask directly: “After I pay this, who tracks my schedule and who files my Clearinghouse reports?”

What do other providers publish for post-SAP pricing?

In July 2026 we reviewed 17 providers in the DOT SAP and C/TPA space to see who publishes post-SAP program pricing. Per that review:

  • Six offer some form of post-SAP or return-to-duty service but publish no price. You have to call for a quote.
  • Six perform SAP evaluations only and refer follow-up testing management to a C/TPA. Their published fees, typically $400 to $500, are for the evaluation, which is a separate cost from testing.
  • One competitor publishes a follow-up enrollment fee (goMDnow, above). Almost nobody publishes an annual managed program price. Vertical Identity publishes a flat annual management fee on its Post-SAP Management Program page.

This is a July 2026 snapshot of published pages, and providers change pricing without notice. The practical point for a driver budgeting one to five years: a price you have to phone in for can change depending on who is asking, and you cannot compare what you cannot see.

What does a six-test year cost side by side?

Your SAP must prescribe at least six unannounced follow-up tests in the first 12 months of safety-sensitive duty (49 CFR §40.307). Using published rates, six observed follow-up tests cost:

ProviderPer observed follow-up testSix tests
Vertical Identity$119 (observation surcharge of $50 is part of the price)$714
goMDnow, non-member (per their published pricing as of July 2026)$134.95$809.70
goMDnow, member (per their published pricing as of July 2026)$124.95, plus $99.00 per year membership$848.70 including the membership

The goMDnow figures leave out their $134.95 follow-up enrollment fee, and none of the figures include the return-to-duty test or any later years if the plan is extended. On the return-to-duty test itself, Vertical Identity’s observed test is $119, and goMDnow lists a $159.95 return-to-duty bundle (per their published pricing as of July 2026, verify when you buy).

We have not restated the difference between providers as a single dollar figure here, because the providers package setup, membership and Clearinghouse reporting differently. Put each provider’s numbers into the checklist in the section on questions to ask and total them yourself.

How does Vertical Identity price it?

Two published numbers:

  • $240 per year for program management. We hold your SAP’s follow-up plan, build and run the unannounced schedule, coordinate collections at sites nationwide, handle MRO review, file your Clearinghouse reports, and keep your records audit-ready.
  • $119 per observed test, for the return-to-duty test and for each follow-up test. That is the $69 base DOT drug test plus a $50 observation surcharge.

A breath alcohol test, if your SAP requires one, is $59. A DOT physical, if your medical card lapsed while you were out of service, is $129. You pay per test as each one happens. There is no prepayment.

For a typical first year, the program fee plus the return-to-duty test plus six follow-up tests comes to $1,073. Your own total can be higher if the SAP prescribes more tests or extends the plan.

See how the Post-SAP Management Program works and the details of the observed return-to-duty test.

What do the federal rules require?

The cost depends on rules you do not control, so it helps to know them:

  • Direct observation (§40.67). Return-to-duty and follow-up urine collections are directly observed. Refusing observation is a refusal to test.
  • Follow-up plan (§40.307). The SAP, not the employer or the provider, decides the number and frequency of follow-up tests. The minimum is six in the first 12 months. The SAP may extend follow-up testing for up to 60 months in total.
  • Return-to-duty process (§40.305). The return-to-duty process concludes only after the driver completes the SAP’s recommended education or treatment and passes a return-to-duty test with a verified negative result.
  • Unannounced tests. Follow-up tests are unannounced, which is why a third party tracking the schedule matters.
  • Clearinghouse. Driver status and return-to-duty progress are recorded in the FMCSA Drug and Alcohol Clearinghouse. A positive result or refusal stays on record until the return-to-duty process and follow-up plan are complete.
  • A third party must run it. Our program page explains that the driver cannot administer their own follow-up schedule; an employer’s designated representative (DER) or a C/TPA must.

A worked example: the first 12 months

Take a driver whose SAP prescribes the minimum plan: one return-to-duty test, then six follow-up tests over 12 months. Here is what a Vertical Identity bill looks like:

LineAmount
Post-SAP program fee (one year)$240
Observed return-to-duty test$119
Six observed follow-up tests$714
First-year total$1,073

Now change the facts. If the SAP prescribes more than six tests, each extra test adds $119. If the plan runs into year two, the program fee renews annually. If the SAP requires a breath alcohol test at some collections, add $59 for each. The SAP evaluation, and any treatment, are billed by those providers and are not in this total.

What should you ask any provider before paying?

Get these answered in writing:

  1. “Is the observed collection fee included in your per-test price, or extra?” If extra, add it to every test.
  2. “Is your program fee one-time or annual?” If one-time, what happens in year two if my SAP extends the plan?
  3. “Do I need a separate membership to get your advertised rate?” Some advertised prices are member-only.
  4. “Who tracks my schedule, you or me?” Setup is not management.
  5. “Who files my Clearinghouse reports, and is that in the price?”
  6. “What happens if I change employers mid-plan?” The follow-up plan belongs to your SAP’s plan and continues, so your program should not have to restart.
  7. “What does a breath alcohol test or DOT physical cost if I need one?”
  8. “Can I see all of this on your website without calling?”

A provider that will not answer these in writing is giving you information too.

Frequently Asked Questions

How much does post-SAP follow-up testing cost?

It has two parts, program management and the tests. At Vertical Identity, management is $240 per year and each observed test is $119, which includes the $50 observation surcharge. Your total depends on how many tests your SAP prescribes, with a minimum of six in the first 12 months.

Why is my follow-up test more expensive than the advertised price?

Follow-up tests must be directly observed under 49 CFR §40.67, and some providers list an observed collection fee as a separate add-on. goMDnow, for example, lists a $45.00 add-on (per its published pricing as of July 2026). Since observation applies to every follow-up test, the add-on applies every time.

Do DOT follow-up tests have to be observed?

Yes. Under 49 CFR §40.67, return-to-duty and follow-up urine collections must be directly observed. It is not optional, and refusing observation is treated as a refusal to test.

How many follow-up tests will I have to pay for?

Your SAP must prescribe at least six unannounced tests in your first 12 months of safety-sensitive duty. The SAP may require more and may extend testing for up to 60 months in total. The SAP decides, not the testing provider.

What is the total cost of the first year?

At Vertical Identity the minimum first year is the $240 program fee, the $119 return-to-duty test and six follow-up tests at $119 each, for $1,073. The SAP evaluation and any treatment are separate and are paid to those providers.

Is the SAP evaluation included in the cost of follow-up testing?

No. The SAP evaluation is a separate service billed by the Substance Abuse Professional. Among the SAP-evaluation-only providers we reviewed in July 2026, published fees were typically $400 to $500. Follow-up testing and program management are billed separately from that fee.

Why do so many providers hide their post-SAP pricing?

We cannot speak for them. Of 17 providers we reviewed in July 2026, six offered post-SAP or return-to-duty services without publishing a price, and six only perform SAP evaluations. Vertical Identity publishes a flat annual fee and a flat per-test price.

Can I manage my own follow-up testing to save the program fee?

No. 49 CFR Part 40 Subpart O requires a third party, either an employer’s designated representative or a C/TPA, to administer the return-to-duty program. This applies to owner-operators too, so someone must run it, and the question is who.

What happens if I miss a follow-up test?

Failing to appear for a directed test is a refusal under 49 CFR §40.191, which carries the same consequences as a positive result. Your return-to-duty status would be affected and the process would restart. This is why having a provider that tracks the unannounced schedule matters.

Does my program cost restart if I change employers?

It should not. The follow-up plan belongs to your SAP’s plan and continues with a new employer. If Vertical Identity manages your program, your new employer designates us and the schedule carries on, with no additional program fee beyond your annual rate.

Do I need a company or a DOT number to enroll?

No. You do not need an employer or a DOT number to enroll in the Post-SAP program, so the schedule can be running before you are back at work.
